Community Outreach
At One Love Center for Health, we believe that healthcare extends beyond the clinic walls. Our outreach programs meet people where they are in schools, on the streets, and within the communities that need care the most. Through trusted partnerships and compassionate service, we bridge gaps in access and support whole person well being.
Street Medicine & Mobile Outreach
Our clinicians and volunteers deliver care directly to people experiencing homelessness and housing insecurity across Solano and Alameda Counties. Services include primary care, wound care, behavioral health support, medication-assisted treatment, and linkage to long term care.
Transitional Housing & Reentry Support
We partner with community-based organizations to provide consistent healthcare and follow-up for individuals living in transitional housing or reentering the community after incarceration.
School & Youth Health Programs
In partnership with local schools, we provide on-site health clinics and sports physicals—improving access to preventive care for students and helping them stay active and healthy.
Behavioral Health & Recovery Partnerships
We provide integrated behavioral health services in partnership with 4th Second, offering access to a Licensed Clinical Social Worker (LCSW) for counseling and care coordination. Our clinical team also delivers comprehensive medication-assisted treatment (MAT) for substance use disorders, including buprenorphine and naloxone for individuals seeking recovery from opioid dependence.
Training & Volunteer Engagement
We train and mentor the next generation of healthcare professionals, including Family Nurse Practitioner students, residents, and volunteers. These experiences help build a workforce rooted in health equity, compassion, and service to the community.
